Patient Forms

Hours

Our hours are from 8:00 a.m. until 5:00 p.m., Monday through Friday. If you have an emergency after hours, call our office and the 24-hour answering service will contact the physician on call. For individual office numbers (click to view).

Appointments

Your physician's nurse will schedule you with the earliest available appointment at a time convenient for you. If you are having a serious problem, we will do everything possible to see you promptly. Please remember that emergency cases may disrupt the schedule and cause a delay in the reception room. However, we try to see everyone at their scheduled appointment time. You can help by being prompt for your appointment.

Your care can be improved by letting the nurse know of any special needs (hard of hearing, disability, or other significant illness).

Insurance Information

We accept Medicare assignment and most insurance plans. Please bring your insurance cards so we may determine if your insurance company requires precertification, referral or a second surgical opinion. If you have any questions, please feel free to contact our office.

We will file your insurance claim for you, however you are responsible for the unpaid balance or charges which your insurance does not pay. You will be notified of all charges every 30 days.

If you are having a billing problem, you may direct your questions to your physician's account representative.

Problems or Concerns

We are here to serve you with helpfulness and courtesy. If you ever have a concern, or perhaps even suggestions on how to serve you better, we would appreciate your comments. You can contact our Administrator at any time.

Waiting Room and Family

If you are having a special procedure, your family members will be updated on your progress while you are in the recovery room. Once you are awake, they will be invited back to be with you. The physician will talk to you and your family in our step down room.

Check-In

Please sign in and notify the receptionist of your arrival. Any change in residency, telephone number, or insurance information should be brought to her attention at that time. You will be asked to complete a health questionnaire.
